Requires all CAE Counsel for all Eligible Claimants for which they are Primary Counsel and all Eligible Pro Se Claimants to provide information to the Court in the format set forth below by no later than September 12th (the “Reference Date” in the MSA), regardless of intent to participate in the Settlement Program.
